Your information
Privacy Policy
Kalgi Care handles personal and health information in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
What we collect
We only collect what we need to deliver safe, appropriate supports. Depending on your relationship with us, that may include:
- Name, contact details, address and date of birth
- NDIS participant number, plan details and plan management arrangements
- Health information — diagnoses, medications, allergies, care needs, clinical notes and assessments
- Emergency contacts, nominees, guardians and treating practitioners
- Cultural, language and communication preferences
- Records of the supports delivered, incidents, feedback and complaints
- For job applicants: qualifications, employment history, screening clearances and referee information
How we collect it
Usually directly from you, or from someone you have authorised to act for you. We may also receive information from the NDIA, your plan manager or support coordinator, hospitals, GPs and allied health professionals — with your consent, or where the law permits or requires it.
Why we collect it
- To assess whether we can safely meet your needs
- To plan, deliver, document and review your supports
- To meet our obligations as a registered NDIS provider, including incident reporting
- To invoice the NDIA, your plan manager or you
- To respond to feedback and complaints
- To recruit, screen and employ staff
Who we share it with
We share your information only where it is necessary and lawful. That may include your treating health professionals, your plan manager or support coordinator, the NDIA, and the NDIS Quality and Safeguards Commission. We may also disclose information where required by law, or where there is a serious and imminent threat to someone’s life, health or safety.
We do not sell your information, and we do not use it for marketing without your express consent.
How we protect it
Records are held in secure, access-controlled systems. Access is limited to staff who need it to do their job. Paper records, where they exist, are stored in locked storage. Staff are trained in privacy obligations and are bound by confidentiality as a condition of employment.
How long we keep it
We retain participant records for the periods required by Queensland and Commonwealth law and by our NDIS registration obligations, after which records are securely destroyed or de-identified.
Accessing and correcting your information
You can ask to see the information we hold about you, and ask us to correct anything that is wrong. Contact us on 0421 969 429 or kalgicare@gmail.com. We will respond within 30 days, and if we cannot give you access we will explain why in writing.
